If you are a dental professional, such as an orthodontist, we ask for a variety of information if you request information about our Services or register to use our Services, including your name, email address, practice address, phone number, and information regarding your orthodontic practice.
Potential Patient Information. If you would like to request information about local providers of our orthodontic devices, we may collect your name, email address, state, and zip code.
Patient Information. If you are a dental professional, such as an orthodontist, and you use our Services to provide your patients with an orthodontic device, we will collect information you provide us about your patients. This information may include patient names, medical information, such as x-rays, photographs, measurements and scans.
Communications. If you contact us directly, we may receive additional information about you. For example, when you contact us for customer service, we may receive your name, email address, phone number, practice information, the contents of a message or attachments that you may send to us, and other information you choose to provide.

Analytics Partners. We use analytics services such as Google Analytics to collect and process certain analytics data. These services may also collect information about your use of other websites, apps, and online resources. 11.1 Changes. We may revise these terms from time to time. If we make any substantial changes, we will notify you by prominently posting notice of the changes on our Site.
